Position Overview

In this outpatient role, you'll manage a dedicated panel of patients, performing assessments, diagnoses, and prescribing treatments. Collaborate with a multidisciplinary team to deliver holistic care, utilizing resources like live interpretation services and remote patient monitoring. You will maintain detailed patient records and leverage telehealth services to enhance accessibility and efficiency.

Position Highlights

✓ Flexible scheduling: 8 AM to 7 PM, Monday-Thursday; 8 AM to 5 PM, Friday

✓ Option for four ten-hour shifts or an 11 AM to 7 PM schedule

Ideal Candidate

You are a passionate healthcare provider with at least two years of experience in a family medicine setting. Experience in a Federally Qualified Health Center (FQHC) is preferred. Bilingual skills in Spanish or Portuguese are highly valued, though not required. You thrive in a team-based setting and are committed to delivering holistic patient care.
