Position Overview

As a Family Nurse Practitioner in this 100% outpatient setting, you will independently manage a diverse patient population across the lifespan. Your role involves chronic disease management, acute visits, and wellness care. You’ll work alongside a collaborative team of medical and behavioral health professionals, utilizing an integrated care approach to enhance patient outcomes. The clinic operates Monday through Friday with extended hours on Thursdays, offering a flexible four-day work week.
