Hospital medicine is defined by its schedule. Seven-on, seven-off is the standard, but block length, shift length, day versus night, swing shifts, and how holidays are handled vary widely, and each pattern attracts a different physician. What to ask about a hospitalist position
The details that decide whether a hospitalist job is sustainable:
- Daily census and admissions per shift, and whether there is a cap.
- Procedures expected, ICU coverage, and whether intensivists or a closed ICU are in place.
- Nocturnist coverage: dedicated nocturnists or rotating nights, and the night differential.
- Backup and surge coverage when census spikes.
- APP support and the supervision model.
- Compensation: base, shift rate, extra-shift pay, and quality bonus structure.
Nocturnist roles
Nocturnist positions pay a premium and usually carry a lighter block schedule, and they are among the hardest for hospitals to fill. If nights suit you, say so; several of our hospital medicine searches are nocturnist-only or offer a nocturnist differential on top of the base.
Compensation and credentialing
Hospitalist pay is usually a base with shift-based extra pay and a quality or productivity component. The salary range at the top of this page is the low and high across current openings and recomputes hourly. Hospital privileging is the timeline driver, commonly 60 to 120 days, and a new state license runs alongside it. Your recruiter manages both from the day you accept.
