Family Medicine and Internal Medicine Salary by State (2026)
Posted compensation on live family medicine and internal medicine physician searches, state by state and by setting, recomputed every hour.
Salary surveys for primary care are published once a year and describe the year before. This page is different: every number on it is a compensation range an employer attached to a search MedStaff Nationwide is running right now, grouped by state and setting, and recomputed hourly. It is smaller than a national survey and far more current, and it shows the one thing surveys cannot: what employers are willing to put in writing today to fill a primary care position.

How to read these numbers
- A range is the low and high an employer attached to a search. The midpoint is the middle of one opening's range, and the median midpoint is the middle of all of them.
- A state figure is published only when at least 3 openings in that state carry a range, so a missing median means the data is thin, not that the state pays less.
- Ranges reflect base compensation. Loan repayment, sign-on bonuses, relocation, and production upside sit on top and are not in these figures.
- Internal medicine here means outpatient and general internal medicine. Hospitalist roles have their own page and typically post higher.

Family medicine versus internal medicine: what actually differs
On the posted numbers the two specialties usually sit close together, and the small gap that appears is mostly setting rather than specialty. Internal medicine includes more hospital-based and inpatient roles, which post higher; family medicine includes more community health center and rural roles, which post a lower base and then add loan repayment that the range does not show. When you compare an FM offer and an IM offer, compare the package, not the range.
What moves a primary care offer above the posted range
- Experience and a portable panel. Physicians who bring patients, or who can build a panel quickly, get offers at or above the top of the range.
- Inpatient scope. Full-scope roles with rounding, nursery, or ED coverage post higher than clinic-only roles in the same market.
- Geography. Rural and Midwestern openings post the highest base; coastal metros post lower and compete on lifestyle and benefits.
- Comp model. A wRVU model with a realistic threshold can pay well above a fixed salary for a productive physician, and well below it for a physician building from zero. The guarantee period decides which.
- How long the search has been open. Older searches move on base, bonus, and relocation together.
