Be part of a team that provides trauma-informed medical and behavioral healthcare to those experiencing homelessness, with opportunities to practice both clinic-based and street medicine.

Position Overview

As a Family Nurse Practitioner, you will provide comprehensive care to adults experiencing homelessness, addressing both acute and chronic medical conditions alongside behavioral health and substance use needs. Your role will involve both clinic care and community outreach to local shelters, always supported by an interdisciplinary team. The schedule is Monday through Friday, ensuring a balanced work-life.

Position Highlights

✓ Salary: $120,000–$149,075 annually

✓ Full-time, Monday–Friday, 8:30 AM–5:00 PM

✓ Combination of clinic-based and community/shelter-based care

✓ Providers never conduct community outreach alone

✓ Interdisciplinary support including behavioral health and case management

✓ Opportunity to practice street medicine

✓ 4 weeks of vacation, 24 hours of personal time, 56 hours of sick time

Ideal Candidate

This role is perfect for a Family Nurse Practitioner with a passion for underserved medicine and experience with complex patient populations. Ideal candidates have at least 2 years of experience, particularly with homelessness, substance use disorders, or behavioral health. PMHNPs with relevant experience are also encouraged to apply.
