Position Overview
As a Nurse Practitioner, you will deliver comprehensive healthcare services to families, including assessments, diagnoses, and treatment plans. Working 24 hours a week, you'll primarily focus on refilling medications for unassigned patients, supporting a mission-driven approach to integrated primary health care. Your daily interactions will be guided by a dedicated Director of Population Health, and you'll work both onsite and through telehealth.
